My issues occured with an Outlook/Exchange 2007 environment. An email with a large attachment (over 100MB) was stuck in the message queue. Message was repeatedly deleted from queue but keeps coming back.

Needed to pause transport queues on all Exchange clustered email servers and rename active queue directories within the local file system. After restarting the transport queues, normal mail flow resumed.

Note: using this proceedure, there was no loss of email.
